Aims of Seaholm Surgery

To provide you and your family with the best possible service and medical care.
To provide a friendly, personal and effective health care. We endeavour to treat all our patients fairly to meet their need in an appropriate way and as promptly as possible.
Our staff should treat everyone courteously and we would ask that patients help us by being polite when they contact us at the surgery and at reception.

Registering

We can currently accept patients with medical cards at the Doctors approval . You can register by attaining a form from the Reception.

Practice Lists are currently open to patients living within our practice area.

You can register by completing a form available from reception. Proof of address and identity are required before registration process can take place. Details of acceptable documents are available from reception.

If you have a medical card with another GP you will be asked to sign a transfer form to have your number transferred to this practice. Also, you will need to sign a letter of authorisation to obtain your medical records from your previous GP. You may be asked to undertake a new patient health check with your new GP or Practice Nurse. New patients may be asked to undertake a review with the Doctor to see if the patients needs are suited to the practice .

 

Medical Cards

Seaholm does accept medical Card patients at the Doctors approval. Medical card froms can be downloaded on www.medicalcard.ie
